Please make checks payable to "EHK Sports" and mail back by June 15th, 2004 or contact us at 847-549-0833!  Deposit of 1/2 registration fee is due with application!   ***Note a $25 Fee will be accessed for all Returned Checks!

Waiver

The participant and his/her guardian agree that EHK Sports Enterprises, our instructors, staff, sponsors and management will not be held responsible for any accidents or loss, however caused, and agree to release the proprietors and management of EHK Sports Enterprises for all claims or damages which may arise as a result of, or reason of such accident or loss.

Cancellation and Refunds

Refunds will not be issued to teams who cancel within six (6) weeks of the tournament beginning. A $500 application and processing fee will be assessed to all applicants requesting and granted a refund no matter what the reason.

EHK Sports Enterprises reserves the right to change the time or location of any tournament.

Confirmation letters will be mailed within four weeks of our receiving registration and payment.
